Onco-core-VariantTested-extension is the same as onco-core-GeneticVariantFound - MCODE #29

      In discussion with Bret and Nephi, per discussion with Lloyd, it should be possible for a given Observation-based profile to not have a derived-from relationship with Observation-based profiles but ensuring that created instances will validate with the dependent profiles (i.e.: "just have the right element stuff").

      This will allow GeneticVariantTested and GeneticVariantFound to be compatible with the CG IG ObsVariant without deriving from it, as long as the FHIR instances created from the mCODE profiles validate against CG ObsVariant profile.

      If accepted, mCODE would need to create a conformance statement that specifies that it validates against the CG IG.

      The onco-core-VariantTested-extension extension can be represented as http://hl7.org/fhir/us/mcode/2019SEP/StructureDefinition-onco-core-GeneticVariantFound. Recommend including an element with required cardinality that required a related observation to http://hl7.org/fhir/us/mcode/2019SEP/StructureDefinition-onco-core-GeneticVariantFound, in http://hl7.org/fhir/us/mcode/2019SEP/StructureDefinition-onco-core-GeneticVariantTested. This would be cleaner from a data retrieval perspective and a better model. Implementation is just as easy as the extension model, if not easier.
