We are aware that the date of death is not usually in the EHR. In the US, death reporting involving funeral directors, coroners or medical examiners, and physician certifiers results in deaths being relayed through state systems to the National Center for Health Statistics, and Social Security Administration. The best choice is one of these two sources.

As an aside, there is two current project ("Death of FHIR" and "Nightingale") is which is FHIR-enabling death reporting.
