      The data conveyed here will not typically represent a 'true' CarePlan, but rather a payer's approximation of a CarePlan based on its particular knowledge of data from claims, prior authorizations and any clinical data shared with it. Payers do not (currently) take on the responsibility of directing clinical care decisions, merely deciding what they will and will not pay for. The purpose of this exchange is to help expidite the decision-making process of the new payer deciding what they will and won't pay for - and minimizing the number of hoops the patient needs to jump through to continue the therapies that were previously covered. As such, reconciliation is unlikely to occur - and if it were to occur, is completely outside the scope of the IG.

      Are the FHIR Care Plan(s) expected to be reconciled? If so, who would perform that action? Should the IG be enhanced with a discussion of reconciled plans? If the plan was transmitted by a prior payer and reconciled but not altered, would any additional notation be appropriate other than typical provenance?
