This use-case does not involve any engagement by providers - it's purely an interaction between payers (with some degree of coordination by the patient).  If the patient and/or the new payer have reason to believe that there is information related to the coverage of active chronic or acute conditions, then that's sufficient to have the new payer initiate the request.  No need to add an additional pre-condition.
