      Determination of a patient's immunization status requires knowing the entire vaccination history from birth. There are no clinic scenarios for patients where it is not necessary to have the full vaccination history. (For certain vaccines, such as flu it might be okay to only know if a flu shot was given this year, but it is still necessary to know what other vaccinations were received so proper recommendations can be made. )
