      We will leave subject as 1..1.  However, we will add guidance noting the ability to specify a contained 'minimal/empty' Patient in the "awaiting extraction" use-case and also provide a recommendation to:

      a) include an identifier on the questionnaire to simplify subsequent retrieval; and

      b) limit read and search permissions for the instance to the author or explicit grantees to avoid exposing patient information for a QuestionnaireResponse instance that is not yet tied to a patient compartment.

      Proposal to relax the constraint on SDC QuestionnaireResponse.subject from "mandatory" to just "mustSupport" and provide usage notes that it should only be absent when context will allow the Patient to be asserted later in the process.

      Rationale: There are cases when implementer may be generating the Patient resource via extraction from the QuestionnaireResponse. Extraction should not occur until the QR is submitted. While the QR is in-progress, the subject Resource would not yet be created, and so the field should not be mandatory.
