There isn't a terribly good reason to have this content sitting in the notes section of the base Questionnaire profile.  It's going to be more intuitive for most readers to learn about translations in the section on rendering and having it exposed directly in the rendering section will increase visibility.  So we'll move that section out of the notes for the Questionnaire profile and put it in the section on Rendering instead.  We'll also update the section to refer to the new 'translation' extension rather than the old iso-21090 extensions.  We'll also refer to the newly added support for translations using ConceptMap.
