The derivation is correct.  However, the description of the profile sucks.  Will change the wording from "Defines how Questionnaire is used to reflect form definitions to be used within the Structured Data Capture standard." to "Describes the elements that should be exposed in a response to a search for adaptive questionnaires".  Will similarly change the description on the base search profile to: "Describes the elements that should be exposed in a response to a search for questionnaires".
