There needs to be sufficient documentation, after removing the notes to balloters, to inform the reader to the purpose and contect of the hook. For example it is referred to as a patient-view hook and yet the example is for an object called 'Context'. It is also not clear is the Context something which is returned following an operation or somethin which is submitted to set the Context or perhaps both.

Please add text to the top of the page to inform the user as to the purpose of the hook and the allowable uses of the hook.
