Consider adding the ability to detect and issue a warning about potentially duplicate extension definitions to the IG publication tooling.

So as part of processing an extension definition, the publication tooling could detect that an extension on the same context of the same type and with a similar description already exists and issue a warning.

IG authors would then be able to "suppress" that warning with a description of why they are defining a different extension rather than reusing the existing definition.
