Clarify point of "period" in observationLinkPeriod for extraction

      Will revamp the definition of the observationLinkPeriod to only apply to population.  Will define a new boolean extension called observationExtract defineable either at the Questionnaire root or on an individual question or group item (not a display item) that indicates that the observation approach should be used to extract either that particular item (based on the code present) or all items in the questionnaire (if they have a code present).

      The SDC extraction page has the sentence "[obsevationLinkPeriod]  indicates the period of time over which to search for matching observations."  That is not relevant for extraction.  Moreover, unless one is aware of the prepopulation use-case for observationLinkPeriod, one wonders what the point is of specifying a period in observationLinkPeriod.
