Enhance value set definition for US Core Procedure Codes

      The SNOMED 129125009 | Procedure with explicit context (situation) hierarchy has too many codes that would introduce variability in how thing are represented in FHIR Procedure (affecting other resource/profiles as well). For example: 

      • Procedure refused (183944003)
      • Procedure not done (416237000)
      • Procedure stopped (394908001)
      • etc.

      When the concept focus is the medical record, such states/situations, are already covered in procedure structural elements and vocabulary binding such as found in  Procedure.status, Procedure.statusReason

      If this hierarchy was added, the logic rules would have to be developed/described to handle this likley redundancy
