      Improve documentation to (1) clarify the role of FSH versus SUSHI versus the IG Publisher, and (2) add a section on how to create the conformance resources CapabilityStatement, OperationDefinition, SearchParameter, and CompartmentDefinition. This has been added in the "Defining Instances" section.

      The Shorthand specification discusses profiles, valuesets, codesystems but is silent on implementation guides. The introduction states that Shorthand is intended for IG generation. We should restate the purpose or add the IG specific elements from SUSHI to the Shorthand specification.
      I propose the rescope Shorthand to profiles, valuesets, etc. and add the IG part in STU2, as this probably requires substantial extensions to the spec.
