      I suggest to lighten the wording.

      Existing Wording:

      i.e., whether the clinical care ultimately delivered recommended care process activities that addressed the intent of the guideline and if it achieved the desired end or intermediate outcomes with consideration for expected (codified in logic) or expressly communicated (documented) exceptions.

      Proposed Wording:

      i.e., whether the clinical care delivered the recommended care process activities and if it achieved the desired outcomes.
