Remove Must Support References to non US Core Profiles

      In all US-Core Profiles that have a Reference to the base FHIR resources and are noted as Must-Support, we will remove the references that are not intended to be Must-Support and include a note to say "the intent is for the listed resources to be Must-Support and the other resources that are listed in the base specification can be sent (even though they may cause a validation error) but are not intended to be Must-Support".

      US Core DocumentReference.author includes references to non-US Core profiles. For example, RelatedPerson/Device/PractitionerRole.

      Please update to reference a specific US Core Profiles or remove the reference here and in other profiles. These additional non-profile references are included in ONCs CCG (https://www.healthit.gov/test-method/standardized-api-patient-and-population-services#test_procedure).
