The Schematron implementation of the dom-3 constraint is incorrect and leads to false-negative validation errors. The solution has already be proposed in the StackOverflow post https://stackoverflow.com/questions/57759578/fhir-r4-contained-referecnes-schematron-validation-fails, so it should just be a matter of applying the fix across all relevant Schematron files.
