For quantity item type, http://build.fhir.org/valueset-item-type.html the definition says "There is an extension 'http://hl7.org/fhir/StructureDefinition/questionnaire-unit' that can be used to define what unit should be captured (or the unit that has a ucum conversion from the provided unit)."
In the extension page http://hl7.org/fhir/extension-questionnaire-unit.html,
the context of use section says: "Use on Element ID Questionnaire.item or Element ID Questionnaire.item.item, with Context Invariant = type='integer' or type='decimal'
So this extension cannot be used on quantity item type.
There is another extension: http://hl7.org/fhir/extension-questionnaire-unitOption.html
which can only be used for quantity item type: "Use on Element ID Questionnaire.item or Element ID Questionnaire.item.item, with Context Invariant = type='quantity'"
So I think this would be the correct extension for quantity.
