Clarify unit used for quantity itemtype

      1. Will correct the extension listed on item type 'quantity' to refer to questionnaire-unitOption and will change the description to say:
        "There are two extensions ('http://hl7.org/fhir/StructureDefinition/questionnaire-unitOption' and 'http://hl7.org/fhir/StructureDefinition/questionnaire-unitValueSet') that can be used to define what unit(s) are allowed to be selected for the Quantity.code/Quantity.system.  "
      2. Will add the following language to both the integer and decimal types:
        "There is an extension 'http://hl7.org/fhir/StructureDefinition/questionnaire-unit' that can be used to computably convey the unit of measure associated with the answer for use when performing data extraction."

      For quantity item type, http://build.fhir.org/valueset-item-type.html the definition says "There is an extension 'http://hl7.org/fhir/StructureDefinition/questionnaire-unit' that can be used to define what unit should be captured (or the unit that has a ucum conversion from the provided unit)."

      In the extension page http://hl7.org/fhir/extension-questionnaire-unit.html,

      the context of use section says:  "Use on Element ID Questionnaire.item or Element ID Questionnaire.item.item, with Context Invariant = type='integer' or type='decimal'

      So this extension cannot be used on quantity item type.

      There is another extension: http://hl7.org/fhir/extension-questionnaire-unitOption.html

      which can only be used for quantity item type: "Use on Element ID Questionnaire.item or Element ID Questionnaire.item.item, with Context Invariant = type='quantity'"

      So I think this would be the correct extension for quantity.
