Extension on DetectedIssue evidence to indicate status of evidence

      Change element evidence/detail from 1..1 Reference(DEQM Individual MeasureReport Profile) to 1..1 Reference(DEQM Individual MeasureReport Profile|GuidanceResponse).

      This allows potential profiling on GuidanceResponse in the future without breaking backward compatibility.
      Change the $care-gap operation status parameter from 0..1 to 1..* and no default. If wants to query both open and closed gaps, then need to provide status parameter for both open and closed.
      Make sure that the binding strength is extensible when referring to the DEQM Gaps In Care Gap Status Value Set from operations and profiles.

      It would be useful to extend DetectedIssue to indicate if the evidence is due to the status (e.g. presence, absence) of a data element. This could be done by creating a small value set for the new element. Other values may also also be useful, though would be more complicated to implement (e.g. lab value out of range, time value out of range).

      For example, in CMS122 we may want to note that the Patient has a present condition of Diabetes and the absence of an HbA1c test.
