Might consider switching the columns so the Report Creation date is before the Gaps through Period Start and End date columns. That way a person reading this knows the current date is 7/1/20 – the date the report was created. Also recommend revising date patient had colonoscopy to be more obvious - e.g. 2010-08-03. Finally, "the colonoscopy would be over 10 years" is unclear.
