      x-fhir-query variables, per https://jira.hl7.org/browse/FHIR-26838, can go to an operation. This raises the question of whether, when doing terminology operations ($lookup or $expand), it should pay attention the terminology server set in the preferredTerminologyServer extension. I would like that answer documented somewhere, perhaps on the preferredTerminologyServer extension page, so that there is consistency between implementations.
