The EHR can act on client messages because - for the EHR to receive messages - the client application must already be running within the same browser context.  In other words, the messages between the EHR and the application do not traverse the network - it's purely window-to-window javascript communication.

There is a an inherent assumption that, receiver application ( EHR client) can act on the message. If it’s a SPA its much easier, however on a traditional web application, it will introduce technical difficulties as it needs to be traveled back to the server to take action.
