Update CPCDS Mapping table in the CareTeam resource intro section with the following changes proposed following a review by the CARIN Blue Button IG Team. 

1. CareTeam:  HL7 instructions to us were to map the billing provider to provider and all associated providers to CareTeam.  The mapping below does not include the billing provider. 
2. a)  Data element name:  Claim billing provider NPI, MapID 94; Claim billing provider name; MapID 167.
3. b)  CareTeam.period:  The .billablePeriod is only used on the IP and OP Institutional claims  Recommend the addition of Dates of service:  ExplanationOfBenefit.item.serviced.servicedPeriod or ExplanationOfBenefit.item.serviced.servicedDate.  Data element names are Service (from) date and Service to date; MapIDs are 90, 119
