Figure 2 is a good example of a simple and very common transition across two care settings. However, it does not complement Figure 1's complexity. Consider adding a use case for three care settings. It is common for patients to move from hospital care to inpatient rehab then to home care. A use case with three care setting will allow implementors to understand the importance of the previous settings data [longitudinal record] and represent the needs of patients and care coordinator teams when more than 2 settings are involved. Understanding the needs of a 3rd setting can be replicated for subsequent settings as illustrated in figure 1
