Allow both answerOption and answerValueSet for an item

      We think it's best to use a contained value set for this purpose because that allows a terminology server to handle the expansion process and it's possible to use existing extensions on the value set concepts to indicate desired sort positions.  Overall, it should minimize work on the client to do things this way.

      We will include an example in SDC that shows how this might work (taking a standard value set and adding a couple of extra concepts like 'other' and 'unknown'.)

      There are situations where the list of codes needs to come from a 'standard' value set but also include a supplemental set of codes.
