      It is implied by the extension, but adding a type is a good idea as there is no system for these identifiers. Will add type from V2 table 0203 https://www.hl7.org/fhir/v2/0203/index.html - these are the codes used in the V2 IG also.

      Should the birth certificate ID extension specify a particular value for Identifier.type or is that assumed because this particular extension is being used? A similar question could be asked for the Birth Certificate File Number extension and the fetal death identifier extensions.
