      The Coded Cause of Death profile requires the fetal death report id extension, but that same extension is not required on the Composition used by the jurisdiction to send to NCHS. It seems like these two cardinalities should be in sync to ensure that the Cause of Death Composition always has an id to return.

      Note that the coded race profile doesn't require a birth or fetal death identifier.
