Should all Observation profiles require .value?

      Some profiles (eg prepregnany weight) have constrained Observation.value to a cardinality of 1..1 but others (eg live births this delivery) leave it at 0..1. Is there a reason for this variability? It seems like .value should always be 1..1. I'm not sure what an Observation resource without a value would be trying to indicate.
