A value set to be expanded might reference one or more 'included' value sets.  In some cases, the terminology server might not have access to these.  (E.g. contained value sets within a Questionnaire where one value sets builds on a base value set.)  There should be a way to pass additional value sets that might be needed as part of the expansion operation.
