MedicationRequest.authoredOn should be optional

      MedicationRequest.authoredOn should be optional or have an invariant to be optional when reported= true.  The patient or recorder of a reported medication or documented/historical medication may not remember or know when a medication was initially authored.

      The date (and perhaps time) when the prescription was initially written or authored on.

      Additionally, a self-prescribed, over the counter medication wouldn't have a prescription or authoredOn date.

      Another option besides making the field optional or having an invariant would be to change the definition to 

      The date (and perhaps time) when the prescription was initially written or authored on or when a reported medication is recorded in the system
