      The ability to capture a reviewed date (and reviewer) is supported by Provenance.  Specifically:

      • Provenance.occurredDateTime (reviewed date)
      • Provenance.agent.who (reviewer)

      when Provenance.agent.type/role is REVIEWER

      and

      Provenance.activity is either PROBLISTREV | DIAGLISTREV

      Something might have an onset date of 3 years ago. But if the status and nothing else has changed, it's possible the record will be unchanged, even though you reviewed it yesterday and confirmed it yesterday. Alternatively, it's possible it hasn't been confirmed in years and is out of date.  It needs to be possible to update the Condition to say "still true as of 'now'" and specify the date when that occurred.  (Condition.recordedDate clearly isn't that based on its definition.)
