      change introduction to say anhy information held by a provider regarding an individual patient.  

      ( can be clinical or administrative )

      To the extent that the Provider keeps a record of the provenance for the source of the data.

      1) Describe how Provenance could be used to source the data  (in other words if administrative or clinical) for Direct Query approach using existing US Core framework, HRexProvenance profile and provide example

      2) Describe how Provenance would be used to source the data  (in other words if administrative or clinical) for Task based approach using:

      1. Search string approach using the Provenance - as above
      2. Using free text  ( i.e. "give me this  data and its provenance" )
      3. for Documents:
        -  assumption is typically implicitly or explicit defined in the Documents themselve so there is no need for an external structure.

      and provide examples of each.
