AECAT and AESCAT don't seem to fit FHIR value set

      AE Category is intended to support a wide varity of different types of categorization, so the AECAT and SCAT values are reasonable to include as well. (Though there is an issue here - see https://jira.hl7.org/browse/FHIR-30501)

      AECAT and AESCAT as being mapped to FHIR's AdverseEvent.category, with the comment "...In FHIR, an AdverseEvent can be categorized in as many ways as desired with whatever granularity is desired." However, AdverseEvent.category refer to "overall type of event", i.e., Quality/'Poduct-use-error/Wrong-dose/…, which has a totally different semantic meaning than the CDISC variables.
