      The Core specification states that "When a FHIR implementation supports any of the vital signs listed below, the implementation SHALL conform to this profile for the vital sign observation." which refers to (among others) the Oxygen saturation profile which uses more general code 2708-6 Oxygen saturation in Arterial blood is inherited from FHIR Core.

       

      It also states: 

      • If a more specific code or another code system is recorded or required, implementers must support both the values (LOINC) listed below and the translated code - e.g. method specific LOINC codes, SNOMED CT concepts, system specific (local) codes.

       

      Thus following this core guidance, this profile is conformant to the FHIR Profile and is more specific defining the method and thus uses the additional coding with the LOINC code 59408-5 Oxygen saturation in Arterial blood by Pulse oximetry.

      Pulse Oximetry specifies the use of an appropriate LOINC code, but then also specifies the use of another, more general code. In the short term this may help some recipients who only understand the more general term, but it seems to provide an inappropriately specific translational service without clear guidelines for where else this sort of thing might be excusable.
