      We will update as follows:

      We have learned that the Mother’s reports (live birth and death) are very specific questionnaires and follow a questionnaire methodology where the order in which the questions are asked of the mother and the exact wording of each question is important:

      • Based on this, we will create Questionnaire instances for the Mother’s Fetal Death Report and Mother’s Live Birth Report
      • Questionnaire will have mappings to the specific profile for each question
      • QuestionnaireResponses could be sent to the Vital Records Office (will be optional in the Composition)

      Note in IG that this architecture is open to change based on pilots/learnings from implementation.

      Building out the submission in this way increases the development overhead necessary for a system to support this reporting method. Using individual data elements and retrieving them from the source EHR would be a better approach and allow reuse of existing FHIR resource support.
