      There may be questions in a Questionnaire that are not associated with a defined SDOH domain and need to be represented as an observation.  The Observation is defined as Social History (required) and Survey (required).  Additionally constraining the observation to include SDOH domain will limit the ability to represent a number of SDOH surveys.  There are know use cases that require more than one domain (e.g. food insecurity do to limited income) and therefore we do not restrict the element to 0..1 or 1..1)

      Observations without an SDOHDomain category value won't be usable in systems that require a domain mapping or don't have a concept of an SDOH need without a domain. Similarly, systems may not know how to interpret observations with multiple domains. Cardinality of SDOHDomain should potentially be 1..1 unless there are known use cases for including multiple domains. Even then at least one should be able to be sent. In addition, this category is marked as "Must support" despite the 0..* cardinality (as is also the case with the other resources in this spec).
