We agree that providing support for SDOH related observations that are not derived directly from a questionnaire is important.  We will add a new observation profile that support user asserted and potentially intervention result asserted observations with a less constrained value set (e.g. allow LOINC and SNOMED-CT)

The concept of an Observation is built around having a questionnaire or screening to go along with it. This makes sense for many use cases, but organizations may also be able to communicate a known need without supporting evidence. Is sending this type of documentation valid, or must a screening and/or questionnaire be included?
