There's a huge amount of assumptions here, and it's not clear why most of them are necessary or relevant - or even why they're assumptions at all. (e.g. assumptions around when appointments are scheduled, use of LOINC, etc.) What is the objective of these assumptions? How are they relevant to software developers creating solutions?
