      There are muliple steps here. First the survey is completed. Then, Observations - including risk scores are generated from the completed survey. Finally a GP leverages those scores together with information gleaned in talking with the patient to record Conditions and add them as concerns to the patient's problem list. Conditions are NOT auto-generated from the survey results.

      Existing Wording:

      The patient interacts with the provider and takes a risk survey that creates the specific health concerns defined by the survey and the patient’s responses.
