      Typically the process is that a Questionnaire response informs the diagnosis of a condition, but there's always a human in the loop that makes the decision - typically with further discussion with the Patient. I think the QuestionnaireResponse can lead to a score Observation, but it shouldn't lead to the auto-creation of a Condition. Suggest removing this as part of the path.
