The Personas and patient stores are great ways to motivate analysis and factors to consider-careful attention should be made and feedgback loops provided to allow enhancement of the understanding of patients, stories and community factors that impact outcomes based on the initial modelling of SDOH with regard to partnering with the healthcare delivery system
