Add OperationDefinition resources to expand the $evaluate-measure and $care-gaps operations to support additional parameters in the same way that the CPG IG specifies OperationDefinition-cpg-cql and OperationDefinition-cpg-library-evaluate.

These references say "Parameter types are mapped to CQL as specified in the Using CQL section of this (CPG) implementation guide. "; so DEQM IG may require a similar content to describe this mapping.
