      The Identifier.type binding is required and uses an X12 value set.  In the FHIR Core specification, Identifier.type already has an extensible binding to a FHIR value set.  That makes the binding in this guide invalid.  We either need to use the FHIR value set and provide a mapping to/from the X12 value set or we need to slice Identifier.type to allow for sending the FHIR code and the X12 code.
