Standardized Core Subscription Topics for Public Health and Other Secondary Data Users

      Public health and other users of population data are very supportive of the FHIR subscription backport and of generally advancing subscription for a number of "secondary data" uses. Consistently implemented, subscriptions can support public health reporting while minimizing provider of care workflow interruptions and EHR system burdens. To be utilized for these purposes, there is a need for some consistent implementation of core subscription topics. There are many possible subscription topics overall, but a standardized core topic set will be needed for secondary data users to develop reliable standards, tools, and software.

      The subscription backport and subscriptions in R5 should include a core set of topics that will be consistently implemented in all EHRs. We propose the use of the "level 0" core topics identified in the US Public Health TriggerDefinition Named Events table available here:

      This list represents a shared, harmonized, topic list that will be used by the electronic Case Reporting and MedMorph reporting activities and standards. We are working to harmonize over 90 different reporting activities so as to minimize provider and EHR burden - with these comments we ask that a reliable core set of subscription topics be advanced in this implementation guide to partner with and support these efforts.
