      We will migrate the two questionnaire 'image' extensions from CPG into SDC and reference them in the 'advanced rendering' profile.  Expectation is that CPG will evolve to have a dependency on SDC and will reference the SDC-defined extensions.

      As part of the migration, we will allow the extensions to also appear on QuestionnaireResponse and will include them in the SDC QR profile as 'optional'.

      Will also provide guidance that meaningful item.text SHALL accompany the image and that renderers SHOULD expose this text as well (e.g. beside the image, as a fly-over, etc.).

      We looked at using the xhtml rendering extension to support specifying images as part of a questionnaire, but found that the approach requires the use of web technology and would have been difficult to implement in an Android native application. As a result we defined two extensions in the cpg-shareablequestionnaire to support specifying the image for a question as well as a response selection.
