There are breaking changes made to Substance that need to be reverted for R4B.  I was under the impression that the scope of R4B was limited and I don't see how these changes were included.

I can understand a change to CodeableReference for Substance.code, but even that is a breaking change.  The rest of the changes should not have happened and have broken my implementation needlessly.
