Clarify Claim & ClaimResponse Extension Descriptions and Definitions

      Need clarity on the Claim and ClaimResponse extension description & definitions (28-35).

      The extension URLs do provide some information/definition, but without an example or additional context it is hard to sync this with corresponding EMR vendor prior authorization models.
