https://jira.hl7.org/browse/FHIR-23028 Was approved in Aug 2019.  The actual implementation was beyond what was approved and this has been in ballots from the R5#1 onwards (but not R4B).  The actual implementation should now be approved.  

It would also be appropriate to change the use of the status attribute to refer just to publication status and avoid possible duplication of states.
