Will clarify the definition of 'required':

If the required item is a question, it must have a direct answer (i.e. an answer to the question itself, not merely answers to child questions) in order for the QuestionnaireResponse to be complete.  If the required item is a group, it must have at least one descendant question which has an answer

The current documentation for item.required=true only says that the item must appear in the QuestionnaireResponse, but that could be satisfied by an answered child item (which forces its unanswered parent to appear). For items that are questions, additional wording is needed to state that they themselves must be answered if they are required.
