Please add text and relationship flow diagrams to explain how a FHIR Care Plan supports the Care Planning Process

      Please embellish the text in Care Plan and add resource relationship diagrams to the the recourse explanatory text under 9.5.2 "Boundaries and Relationships". I believe this is important to add to FHIR 5.0 in order to make this resource understandable to create usable and implementable profiles. Please see FHIR-28546
