Extension definition still very hard to read see example at http://hl7-fhir.github.io/extension-medicationadministration-immunization-reaction.html. lots of stuctural detail clutters out the content of the extension. also the 0..0 on value[x] took me a few minutes to understand. Propose to have a simplified structure definition which hides the extensions and url elements so see content only.
